Craven SPORT Services
After booking your Craven CONNECT @ HOME online physiotherapy appointment, you will be asked to complete our ‘Informed Consent for Telerehabilitation’ form. This form ensures that you are aware of the potential risks involved with online physiotherapy, and that you consent to use communicating via email and video chat prior to and during your appointment.
Clients 18 and over may complete this form for themselves. Clients under 18 are asked to do so alongside a parent or guardian, who will sign on their behalf.
