If this is your first appointment with a Craven SPORT services physiotherapist, you’re treating a new injury, of you haven’t been to our clinic in a while, please fill out the following form and we will contact you to schedule a 1-hour assessment with a physiotherapist that will best meet your needs.
If you have worked with one of our physiotherapists in the past, or would like to book with a particular team member, please indicate so below. Appointment availability may vary according to requested therapist. If you’re not sure who to book with, our Administrative Team will help pair you with a therapist who best suits your needs!

CANCELLATION POLICY: CRAVEN SPORT SERVICES RESERVES THE RIGHT TO CHARGE A CANCELLATION FEE OF $50.00 FOR ONE HOUR MISSED APPOINTMENTS AND $25.00 FOR HALF HOUR MISSED APPOINTMENTS, OR WHEN LESS THAN 24 HOURS CANCELLATION NOTICE IS PROVIDED PRIOR TO A CLIENT’S SCHEDULED APPOINTMENT TIME. AFTER TWO OR MORE LATE CANCELLATIONS/NO SHOWS, THE CLIENT WILL BE REQUIRED TO PAY FOR THE FULL AMOUNT OF THE APPOINTMENT.
